Τι πρέπει να διαβάζουμε σε μια ενδοσκοπική έκθεση όταν υπάρχει οισοφάγος Barrett?

Επειδή ο οισοφάγος Barrett είναι λιγότερο συχνός στην χώρα μας απ'ότι για παράδειγμα στις χώρες της Βορείου Ευρώπης, πολλοί γαστρεντερολόγοι δεν είναι εξοικειωμένοι με την νόσο αυτή. Κατά την ενδοσκοπική περιγραφή πρέπει να αναφέρεται η ύπαρξη ή όχι ολισθαίνουσας διαφραγματοκήλης καθώς και τα οδηγά σημεία δηλαδή

• η γραμμή Ζ [περιοχή που μεταπίπτει το τοίχωμα του οισοφάγου σε τοίχωμα στομάχου]
• το εντύπωμα του διαφράγματος
• το άνω πέρας των γαστρικών πτυχών

Η απόσταση από το άνω πέρας των γαστρικών πτυχών έως την γραμμή Ζ είναι το μήκος του οισοφάγου Barrett.

Eπειδή ο οισοφάγος Barrett δεν είναι πάντα κυκλοτερής, εδώ και λίγα χρόνια πρέπει να εφαρμόζεται από όλους τους γαστρεντερολόγους η ταξινόμηση της Πράγας [Prague classification] στην περιγραφή του. Αυτή έχει 2 αριθμούς CxMy όπου χ το μήκος του κυκλοτερούς και y το μέγιστο μήκος του Barrett. Δηλαδή C3M4 σημαίνει πως ο Barrett έχει μήκος 3 εκ κυκλοτερώς και μια γλωσσίδα άλλο 1 εκ που φτάνει δηλαδή στα 4 εκ.

Tι είναι ο οισοφάγος Barrett?

Ο οισοφάγος Barrett είναι μια αλλαγή στο τοίχωμα του κατώτερου οισοφάγου, κατά την οποία αντικαθίσταται το φυσιολογικό τοίχωμα του οισοφάγου, από τοίχωμα στομάχου. Η αίτια της ανάπτυξης οισοφάγου Βarrett είναι η γαστροοισοφαγική παλινδρόμηση. Το γαστρικό οξύ παλινδρομεί πίσω προς τον οισοφάγο και ερεθίζει το τοίχωμα του. Ο οισοφάγος αντιδρά και αναπτύσσει ένα τοίχωμα ανθεκτικό στο οξύ, μόνο η ανάπτυξη αυτή είναι ελαττωματική γιατί είναι προκαρκινική. 

Νεώτερες ενδοσκοπικές τεχνικές στο Γαστρεντερικό σύστημα: Mεγεθυντική ενδοσκόπηση, Narrow Band Imaging, Autufluorescence endoscopy.

ΣΥΝΤΟΜΟ ΒΙΟΓΡΑΦΙΚΟ ΣΗΜΕΙΩΜΑ

O Γεώργιος Κ Αναγνωστόπουλος γεννήθηκε στην Αθήνα στις 19/3/1970. Αποφοίτησε το 1987 από το Πρότυπο Λύκειο Αναβρύτων και το 1993 πήρε με ‘άριστα’ το πτυχίο της ιατρικής από την Ιατρική Σχολή του Εθνικού και Καποδιστριακού Πανεπιστημίου Αθηνών. Αφού εξέτισε την στρατιωτική του θητεία στο Πολεμικό Ναυτικό και το αγροτικό του στο άγονο περιφερικό ιατρείο της νήσου Ίου, ειδικεύθηκε στην Γαστρεντερολογία στο 251 Γενικό Νοσοκομείο της Αεροπoρίας. Το 1993 έλαβε υποτροφία από την Ελληνική Γαστρεντερολογική Εταιρεία και μετεκπαιδεύθηκε στα Wolfson Digestive Diseases Centre, University of Nottingham UK και Endoscopy department, University of Mainz, Germany στις προηγμένες μικροενδοσκοπικές διαγνωστικές και θεραπευτικές τεχνικές [μεγεθυντική ενδοσκόπηση, χρωμοενδοσκόπηση, Νarrow Band Imaging, confocal endomicroscopy, autofluorescence endoscopy, ενδοσκοπική βλεννογονεκτομή, φωτοδυναμική θεραπεία σε οισοφάγο Barrett’s]. Στην Ελλάδα εργάζεται στον ιδιωτικό τομέα από το 1997 ενώ παράλληλα συνεχίζει το ερευνητικό και κλινικό του έργο.

Kύρια ενδιαφέροντα του είναι η παρακολούθηση των ασθενών με οισοφάγο Barrett’s και ελκώδη κολίτιδα καθώς και η ενδοσκοπική μελέτη ασθενών με προκαρκινικές αλλοιώσεις στο πεπτικό [δυσπλασία]. Είναι ομιλητής σε πολλά ελληνικά και διεθνή συνέδρια ενώ έχει επιδείξει ζωντανά τις προηγμένες ενδοσκοπικές αυτές τεχνικές σε ενδοσκοπικά σεμινάρια τόσο στην Ελλάδα όσο και στο εξωτερικό. Μεγάλος αριθμός των εργασιών του είναι δημοσιευμένες σε έγκριτα peer-reviewed ιατρικά περιοδικά στο εξωτερικό, ενώ έχει περισσότερες από 100 δημοσιεύσεις σε ελληνικά και ξένα συνέδρια.

ΕΙΔΙΚΑ ΕΝΔΙΑΦΕΡΟΝΤΑ

· Οισοφάγος Barrett's και ΓΟΠΝ. Ένα μεγάλο μέρος της ερευνητικής και κλινικής ενδοσκοπικής μου μελέτης και εμπειρίας βασίζεται στην ενδοσκόπηση ασθενών με οισοφάγο Barrett’s με την χρήση των μοντέρνων ενδοσκοπικών τεχνικών που έχω ήδη αναφέρει αλλού.

· Ογκολογία πεπτικού

· Ανεύρεση και ενδοσκοπική θεραπεία του πρώιμου καρκίνου και προκαρκινικών καταστάσεων στο ΓΕΣ. Στον θεραπευτικό τομέα της ενδοσκόπησης ασχολούμαι κυρίως με την αφαίρεση ενδοσκοπικά προκαρκινικών καταστάσεων στο πεπτικό [πχ πολυποδεκτομές] αλλά και στην ενδοσκοπική αφαίρεση του ενδοβλεννογόνιου καρκίνου τόσο στο ανώτερο όσο και στο κατώτερο πεπτικό [βλεννογονεκτομή]

· Προηγμένες ενδοσκοπικές τεχνικές [μεγεθυντική ενδοσκόπηση υψηλής ευκρίνειας, Narrow Band Imaging, χρωμοενδοσκόπηση, autofluorescence endoscopy]

· Ελκώδης κολίτις και νόσος Crohn’s. Οι ασθενείς με ελκώδη κολίτιδα έχουν μετά από κάποια χρόνια νόσου μεγάλη πιθανότητα να εμφανίσουν καρκίνο παχέος εντέρου. Οι ενδοσκοπικές τεχνικές που έχω αναφέρει θα βοηθήσουν στην πρώιμη διάγνωση και αφαίρεση προκαρκινικών καταστάσεων σε αυτούς τους ασθενείς πριν εμφανισθεί καρκίνος
